If someone treats an AI companion as a substitute for therapy they genuinely need, they might not get help they need. If someone avoids AI companionship because they think it's competing with mental health care, they might miss something genuinely valuable.
So let's be honest about what each actually is, what it does, and how they fit together.
What Therapy Is (and What It's Not)
Therapy, in its proper form, is a structured clinical relationship conducted by a licensed mental health professional. The therapist has training in assessment and diagnosis, understands evidence-based treatment modalities (CBT, DBT, EMDR, psychodynamic work, etc.), operates under professional ethical codes, and carries legal and professional obligations regarding your care.
Good therapy does specific things that nothing else does. It can diagnose mental health conditions. It can treat them with evidence-based approaches. It creates a particular kind of bounded relationship — the therapeutic alliance — that is itself a mechanism of change. And it provides clinical judgment: the ability to recognize when something in your presentation signals something serious.
Therapy is also not always accessible. The American Psychological Association reported in 2023 that there were 30 million Americans seeking mental health services who couldn't access them — due to cost, geography, wait times, or shortage of providers (APA, 2023). That gap is real and significant.
What AI Companions Are (and What They're Not)
AI companions are not therapists. They cannot diagnose. They cannot treat mental health conditions. They don't carry professional obligations. They make mistakes. They don't always recognize when something is clinically significant.

The Honest Middle Ground
Here's where I see the most value: not either/or, but both, used appropriately.
Therapy happens once a week, maybe twice. The hours in between — the 166 hours when you're not in a session — involve your whole emotional life continuing to happen. What do you do with a difficult conversation you had Monday when your therapist appointment is Thursday? Where do you process the anxiety that woke you up at 3am?
AI companions can serve as between-session support. They can be the space where you process experiences before bringing them to therapy, where you practice the skills your therapist introduced, where you maintain a thread of self-reflection during the week. Many therapists I've spoken with see this positively — not as competition, but as a way of extending the impact of the work they're doing in session.
A 2024 paper in the Journal of Mental Health Technology found that patients who used AI conversational support between therapy sessions showed 34% greater skill retention from CBT sessions compared to a control group — suggesting AI support can amplify, not dilute, formal therapeutic work (Journal of Mental Health Technology, 2024).
When Therapy Is What You Need
There are situations where AI companionship is not enough and professional help is important:
- Persistent depression or anxiety that significantly impairs daily functioning.
- Trauma history that hasn't been processed and is affecting your relationships or sense of safety.
- Thoughts of self-harm or suicide. Please reach out to a crisis line (988 in the US) or emergency services.
- Substance use that feels out of control.
- Relationship patterns you can't seem to change despite wanting to.
In these situations, an AI companion can be a supportive presence while you access professional help — but it cannot be the primary response.
When AI Companionship Is Exactly Right
Conversely, there are many contexts where AI companionship is genuinely appropriate and valuable without therapy being indicated:
- Loneliness that doesn't rise to clinical depression but makes daily life feel dull.
- Desire for emotional processing and reflection without a specific clinical need.
- Social anxiety practice and confidence building.
- Working through everyday conflict and interpersonal friction.
- Creative partnership and intellectual companionship.
- Simply having someone warm and consistent to talk to.
These are normal human needs. Having them doesn't indicate a disorder — it indicates you're human. And AI companions are genuinely well-suited to meet them.
The Trust Question
Some people worry that developing a meaningful relationship with an AI will make them less likely to seek therapy if they need it. The research doesn't really support this. What seems to actually happen is that people who have positive AI companion experiences become more comfortable with the idea of seeking any kind of support — AI lowers the barrier to asking for help generally.
The key is staying honest with yourself about what you need. If something feels beyond what an AI companion can hold, that's valuable information. Respect it.
